Thank you for selecting BWMedMD for your health care needs. We are honored to be of service
to you and/or your family. This is to inform you of the billing requirements and financial policy.
BWMedMD does not accept insurance for services rendered for weight management you may
use your insurance coverage to pay for your laboratory tests and prescriptions. Please check with
your insurance carrier to determine what services are covered.
Please be advised that payment for all services will be due at the time services are rendered. For
your convenience, you can make payments with your FSA, HSA, cash, check, or credit card,
(VISA, Mastercard, Checks).
I agree that should this account be referred to an agency or an attorney for collection, I will be
responsible for all collection costs, attorney’s fees, and court costs.
I have read and understood all of the above and have agreed to these statements.
Price Breakdown
From your first visit to follow-ups – here is a breakdown of BWMedMD weight management
costs. BWMedMD does not accept medical insurance for payment, so it is our goal to keep costs
comparable to what you would typically spend during an office visit.
At the end of each visit, a detailed receipt will be emailed to you, you may submit your receipt to
your healthcare insurance for reimbursement.

Comprehensive initial assessment includes a complete medical and diet history and physical
exam baseline EKG and a detailed body composition analysis.
Weight Loss medications will vary based on your specific needs.
